One-on-One Physical Therapy

Physical Therapy for Singers, Speakers, and Vocal Performers in Baltimore

Dr. Maks Birikov, DPT is the only practitioner in Baltimore — and to his knowledge, in Maryland — who specializes in the physical demands of singers and vocal performers. Opera singers traveling from Europe to perform in Baltimore seek him out specifically. That is not a marketing claim. It is what happens when there is no one else.

Every session is one hour of hands-on manual therapy with Dr. Birikov directly. No aides, no assistants, no handoffs. He works the jaw, the tongue, the neck, the thoracic spine, and the diaphragm — the full anatomical chain that governs your voice — because treating only one part of it is why most singers stay stuck.

Out-of-network by design. HSA and FSA accepted.

The Format

The Only Practitioner in Baltimore Who Understands What Singers Actually Need

Most physical therapists treat singers the same way they treat anyone with neck pain or jaw tension. They address the site of the complaint and send you home with exercises. That approach misses the point entirely, because a singer's body is an instrument — and the instrument has a chain.

Dr. Birikov has worked with opera singers at the professional level, including performers who travel internationally and sought him out while in Baltimore specifically for this work. That kind of referral does not happen by accident. It happens because the results are real and because no comparable option exists in the region.

If you have been told your vocal problem is a technique issue, a hydration issue, or something to manage rather than fix, there is a good chance the physical component has never been properly assessed.

Self-Selection

The Full Chain: Jaw, Tongue, Diaphragm, and Vocal Performance

Here is what most singers have never been shown: the tension in your jaw is directly connected to the tension in your tongue. The tension in your tongue loads the muscles at the base of your skull and down through your cervical spine. That chain continues into your thoracic spine, your ribcage, and ultimately your diaphragm — the engine of your breath support.

This is for you if

  • You want hands-on bodywork: fascia, joint, nerve, needle. Not a program of exercises.
  • You have already tried PT, massage or chiropractic and are still in pain.
  • Your problem is chronic, recurring, or has never been explained to your satisfaction.
  • You would rather have one hour with the doctor than three appointments with three people.
  • You are willing to pay out of pocket for undivided expert attention.

This is not for you if

  • You are looking for an exercise-based rehab program supervised by an aide.
  • You need a clinic that bills your insurance directly and books you three times a week.
  • You have an acute traumatic injury that needs imaging or surgical evaluation first.
  • You want a single visit that makes a decade-old pattern disappear.
Clinical Reasoning

What Therapy for Singers and Speakers Involves

The first session begins with a thorough history: how you use your voice professionally, what symptoms you are experiencing, what you have already tried, and what has not worked. Dr. Birikov then assesses posture, breathing mechanics, jaw mobility, tongue tension, and the movement of your ribcage and thoracic spine.

A shoulder that lives in the rib cage

Treatment begins the same visit. Depending on what the assessment reveals, that may include myofascial release to address restricted connective tissue throughout the jaw, neck, and chest; buccal massage, which works the internal muscles of the jaw and soft palate; dry needling to release deep trigger points in the neck, suboccipital muscles, and diaphragm; and IASTM — instrument-assisted soft tissue work that uses a firm tool to break up adhesions and restore glide in restricted fascia.

Cupping may also be used on the thoracic spine and ribcage to decompress restricted tissue and improve the mobility of the structures surrounding your lungs. Every technique has a specific mechanical purpose. Nothing is added for its own sake.

Scope

Who This Work Is For

This page exists for a specific kind of person. Not every singer or speaker needs this level of intervention. But if any of the following describes you, it is worth a conversation.

Pain & Injury
  • You are a singer, voice actor, public speaker, or performer whose livelihood depends on your voice.
  • You have jaw tension, neck tension, restricted breath, or vocal fatigue that does not resolve with rest.
  • You have worked with a vocal coach and the problem persists — meaning the issue is physical, not technical.
  • You have been to a general PT, a chiropractor, or a massage therapist and gotten temporary relief but no lasting change.
Structure & Fascia
  • You want one hour of direct, hands-on work with a specialist who understands the demands of performance.
  • You are looking for vocal coaching or singing technique instruction — that is outside the scope of this work.
  • You have an acute throat or laryngeal injury that requires ENT evaluation first.
  • You want a single session to resolve a problem that has been building for years.

Dr. Birikov's Experience with Performing Artists

01

History, briefly

Dr. Birikov holds a Doctor of Physical Therapy degree and trained in European fascial rehabilitation methods. He is the first and only Functional Patterns practitioner in Baltimore. His patient base includes professional athletes, post-surgical patients with complex presentations, and performing artists — including opera singers who travel from Europe to perform in Baltimore and seek out his care specifically.

02

Assessment by hand

That last point is worth sitting with. These are professional performers at the highest level, in a city with no shortage of medical options, choosing to see one specific practitioner. The reason is straightforward: no one else in the region is working with the jaw-tongue-diaphragm chain at this level of specificity.

03

Treatment, same visit

Dry needling is performed under Dr. Birikov's Maryland physical therapy license, which authorizes the procedure. His master-level credential in this area is not a weekend certification.

04

An honest arc

Before you leave, an estimate of how many sessions this will take and what will change first. Most patients feel meaningful change within four to six.

Deep manual work can be intense while it is happening and can leave a day or two of soreness, the kind that follows hard exercise rather than injury. Cupping can leave circular marks that fade within a week. Dr. Birikov talks through pressure as he works and adjusts on request. Nobody is asked to endure anything silently.

Good Questions

Book a Session — Baltimore's Specialist for Singers and Speakers

How is one-on-one physical therapy different from PT at a regular clinic?

Most patients with vocal tension and performance-related physical complaints see meaningful change within four to six sessions. Chronic jaw restriction or long-standing postural patterns may take longer. Dr. Birikov will give you an honest estimate after the first session — not a vague range, and not a commitment to an open-ended treatment plan.

How many sessions will I need?

No referral is required. You do not need to be an existing patient. Send a message describing what you are experiencing, how long it has been present, and what you have already tried.

Do you take insurance for one-on-one physical therapy?

Physica Medica is out-of-network. Sessions are paid out of pocket. HSA and FSA cards are accepted. The cost reflects an hour of direct, specialist-level care — not a shared appointment, not a supervised exercise session, and not a treatment model built around insurance reimbursement rates.
