Low back pain is the most common complaint, but it is rarely the whole picture. The lumbar spine is reacting to what is happening at the pelvis, the hips, and the thoracic spine above it. Treating only where it hurts is why relief stays temporary.
Pelvic girdle pain — including sacroiliac joint dysfunction — is one of the most undertreated conditions in pregnancy. The SI joints become hypermobile as relaxin increases, and the surrounding musculature compensates in ways that create deep, aching pain through the pelvis and buttocks. Round ligament pain, the sharp pulling sensation felt in the lower abdomen and groin, is another condition that responds well to manual therapy when the surrounding fascia is addressed directly.
Postural changes that come with a shifting center of gravity — forward head position, increased thoracic kyphosis, anterior pelvic tilt — create their own chain of symptoms: mid-back tension, rib pain, hip flexor tightness, and nerve sensitivity down the leg. These are not separate problems. They are one pattern.
