The person who assesses you is the person who treats you, for the entire hour, every visit. No aide, no technician, no third patient staggered into the same slot.
Most insurance-based clinics cannot deliver this. Reimbursement pays per visit, not per hour of skilled contact, so the model requires volume: ten minutes with the therapist, forty with a band and a printed sheet. That is not a criticism of those clinicians. It is arithmetic, and it is the most likely reason your last course of physical therapy did not hold.
Here the arithmetic is different. Dr. Birikov works, watches the tissue respond, and changes what he is doing in real time. Findings are explained while he is finding them. The same hour that treats you is the hour that tells you what is actually wrong.

Assessment by hand
Standing, walking, breathing, then palpation. He is looking for where the restriction begins, which is rarely where it hurts.
Treatment, same visit
You are treated on day one. Manual release, needling, cupping or instrument work: whatever the findings call for.
An honest arc
Before you leave, an estimate of how many sessions this will take and what will change first. Most patients feel meaningful change within four to six.
Deep manual work can be intense while it is happening and can leave a day or two of soreness, the kind that follows hard exercise rather than injury. Cupping can leave circular marks that fade within a week. Dr. Birikov talks through pressure as he works and adjusts on request. Nobody is asked to endure anything silently.

How is one-on-one physical therapy different from PT at a regular clinic?
At most clinics your hour is shared. Ten minutes of hands-on time with the therapist, the rest with an aide and a printed program. Here the hour belongs to you. It is manual treatment by a doctor of physical therapy from the first minute to the last, and the plan changes as the tissue changes.
How many sessions will I need?
Most patients feel meaningful change within four to six sessions. Recent, localized problems often resolve faster; scoliosis, long-standing fascial restriction and post-surgical adhesion take a longer course. Dr. Birikov gives you a specific range after the first assessment rather than a guess before it.
Do you take insurance for one-on-one physical therapy?
No. Physica Medica is out-of-network by design. Reimbursement rules dictate session length and staffing, which is precisely what makes this format impossible inside them. Sessions are paid out of pocket; HSA and FSA funds are accepted and a superbill is provided for out-of-network claims. Rates are quoted plainly before you book.
